Isabel Orsately and Nicole Ewerth, MPH, BSN, RN Recognized with the Center for Population Health’s Employee Appreciation Award

Care Transition Navigator Nicole Ewerth, MPH, BSN, RN and Community Health Worker II Isabel Orsately recently earned the Temple Center for Population Health’s Employee Appreciation Award, which recognizes team members for outstanding performance and commitment to their patients.

Nicole was singled out for going the extra mile to support her patient and their family. While working with a new mother experiencing post-childbirth complications, Nicole learned that the patient’s mother-in-law—who was staying with the family—was suffering from cancer. 

Nicole educated the mother-in-law about options for cancer care in our area, and she chose to be seen at Fox Chase. Nicole was also able to secure her a new patient appointment at Chestnut Hill Hospital within five days.  

Nicole’s knowledge and dedication ensured the mother-in-law received the care she needed, and kept her patient—the new mom—from experiencing added stress.

Isabel was honored for her extraordinary dedication to her patients, consistently devoting extra time to ensuring they receive the care they need, as this excerpt from her nomination shows:

“Isabel has consistently been praised by our Nurse Navigation team for her exceptional commitment and willingness to go above and beyond. On multiple occasions, she has taken time from her schedule to provide crucial translation services for our Spanish-speaking diabetes patients on the hospital floor. In one instance, she continued to follow up to ensure that a patient received the necessary care and services to manage his diabetes after discharge. Her outstanding performance as a facilitator in our Diabetes Prevention Program has also been invaluable. I can’t thank her enough for her dedication and impact.”
