Temple Health’s patient portal provides users with the ability to access portions of their electronic health record and manage many of their day-to-day healthcare tasks. These portals are available to anyone who has received care at one of our hospitals or from a Temple or Fox Chase physician. There is no charge to use this safe, secure, web-based portal. myTempleHealth is a secure patient portal that lets you view your health information and manage your
care anytime from your phone, tablet, or computer.

Making it Easy to Manage Your Health

Need to schedule a visit or get a prescription refilled? Now you can log into your myTempleHealth account and take care of it online.

Ease for Caregivers and Parents

Managing a loved one’s healthcare has never been easier thanks to myTempleHealth. With proxy access, you can make sure that they are up to date with visits, immunizations and medicines.

Review Your Visit Notes at Home

After your visit, you will be able to see a summary of your visit, your treatment plan, next steps added into your electronic health record by your provider, and view orders placed by your provider.

With myTempleHealth You Can:

• Schedule or reschedule your appointments, request referrals, and view upcoming visits
• See your medicines, immunizations, allergies, and your provider’s note
• See your medical history and test results
• Fill out questionnaires online
• Contact your doctor’s office about non-urgent medical questions
• Ask for prescription refills
• Find and read patient education materials about your medical condition
• Get reminders about tests or immunizations you may need
• Download and send your summary of care to another doctor or to yourself
• View bills, make payments, enroll in paperless billing, and communicate with billing customer service for help understanding your bills
• Check your hospital and emergency visit progress status by following along in real-time within the app

How do I Sign Up?

Patients can sign up for myTempleHealth by visiting my.templehealth.org or on the MyChart app on your
mobile device. Simply click Sign Up and follow the steps to join myTempleHealth.

Patients 13 and older can use the activation code provided on your After Visit Summary or discharge information within 45 days of receiving the code. No Code? No problem! Click the sign up with your information button and complete the enrollment form. Patients under the age of 13 require sign up in their provider’s office.
