HAPOnline Showcases Expansion of Temple’s Social Drivers of Health Screening Initiative in Its Caring for the Commonwealth Series

The Hospital and Healthsystem Association of Pennsylvania (HAP)’s Caring for the Commonwealth series showcased the expansion of Temple’s Social Drivers of Health (SDOH) screening initiative across primary care, emergency departments and specialty settings. The screening initiative has helped identify barriers related to housing, food security, transportation, financial stability and access to care. This, in turn, enables earlier intervention and stronger care coordination.
