HAP Online highlighted Temple Health receiving the Living the Vision award as part of HAP’s (the Hospital and Healthsystem Association of Pennsylvania’s) annual Achievement Awards, which honor hospitals and health systems for their innovation, creativity and commitment to patient care. Temple Health was recognized with the Living the Vision award for its project to improve health outcomes and health equity by identifying and addressing barriers to patients’ health. The project was led by Nina O’Connor, MD, FAAHPM, Chief Medical Officer of the Temple Center for Population Health; Steven Carson, MHA, BSN, RN, Senior Vice President of Population Health for Temple Health; and Nancy Porter, MPH, BSN, RN, Project Manager for the Temple Center for Population Health.
