In-house benefits that provide a free eye exam, frames and lenses every two years for employee, spouse, and other eligible dependent children.
Free to employee. Amount of coverage $10,000.
Optional. Employee purchases amounts in 1.5, 2, 3, 4, or 5 times annual base salary up to $1 million. Medical questions will apply for coverage requests over 3 times pay and/or $500,000+.
Optional. Employee pays portion of the premium for coverage equaling 60% of base monthly salary up to $10,000/month. Benefit elimination period is the later of 6 months or the exhaustion of sick/PTO time.
Optional. Employee may purchase 60% replacement income to be used in the event of an illness or off the job injury. Benefit elimination period is the later of 30 days or the exhaustion of paid time.
Optional. Pre-tax employee contributions for health, dental or vision care expenses up to $2,650 annually. Benefit runs on a calendar year basis.
Optional. Employee and dependent coverage available first day of month after enrollment.

**If you are employed under an 1199C contract, you will have the option to elect benefits with TUHS or 1199C. If you choose the TUHS benefit package you will be enrolled in the Advantage Medical Plan and will be eligible for the other benefits explained here.
