At our Northeast: Cottman Avenue location, our providers are certified to perform commercial driver’s license (CDL) exams. For CDL exams, please bring:

• A list of all the prescription and over-the-counter medicines you take, including strength and dosage
• Contact information for your doctors
• Patients who have had a heart attack or cardiovascular disease need to bring a copy of your last stress test
• Sleep test results for patients with sleep apnea
