Signing Up for myTempleHealth

Signing Up for myTempleHealth: Your Electronic Health Record

myTempleHealth is a website and mobile app that allows you to access portions of your outpatient electronic health record from your computer, tablet or mobile device. This means you can view your medical information from anywhere — your home, work or even while traveling.

Request myTempleHealth Account Activation Online

You can register online by visiting my.TempleHealth.org, clicking on the new user sign-up and completing the Request myTempleHealth Account Activation form. Follow these steps:

  1. Go to my.TempleHealth.org and click “Sign Up Now.”
  2. On the next screen, click “Sign Up Online” to get to the “Request myTempleHealth Account Activation” page.
  3. Complete all fields on the Request myTempleHealth Account Activation form. Click “Next”.
  4. You will be directed to LexisNexis to verify your identity with 3 questions. Click “Next” after each answered question. Passing verification allows you to create your account.
  5. Enter a username and password then reenter the password. Click “Next”.
  6. Set your email notifications and confirm your email address. Click “Sign in”.
  7. Accept the Terms and Conditions.
  8. You are now active on myTempleHealth.
  9. If you fail verification, we will contact you within 7 business days to resolve the problem.

Activate Your myTempleHealth Account

You may have received a letter, After Visit Summary or Discharge Summary with your myTempleHealth activation code. Follow these steps to activate your account:

  1. Go to my.TempleHealth.org and click “Sign Up Now.”
  2. Enter your myTempleHealth activation code exactly as it appears on your enrollment letter, After Visit Summary or Discharge Summary (you won’t need the access code after this).
  3. Enter your primary phone number and your birthdate, then click “Next”.
  4. Create your myTempleHealth username and password then reenter the password. Click “Next”.
  5. Set your email notifications and confirm your email address. Click “Sign in”.
  6. Accept the Terms and Conditions and you are now active on myTempleHealth.

With an active myTempleHealth account you have the option to download the myChart mobile app. The myTempleHealth mobile app makes it easy to access your outpatient electronic health record wherever you are.

  • iPhone® users can download the myTempleHealth mobile app by searching “MyChart” in the App Store.
  • Android users can download the myTempleHealth mobile application by searching “MyChart” in the Google Play Store.
    1. Once downloaded search Temple for the app to find myTempleHealth
    2. Click the myTempleHealth logo
    3. Enter your username and password
    4. You are now active on myTempleHealth Mobile
